Registration Forms and Payment for Current Students Only

This page is for current students to register for Scrollworks Summer 2012.

*Please note:

-You should expect an email confirming receipt of your application within 3 days. If you have not received such notification, please email. Include your child's name. (Please give us a few extra days this first week of April as Johnathan is out of the office until Thursday and Jeane is out of the office beginning Thursday!)

-To make sure we get your email on our list, please sign up for the newsletter using the box in the right sidebar. If you use Facebook or Twitter, connect with Scrollworks there, too. 

-Please tell everyone about Scrollworks. We want every child to have the opportunity to learn an instrument! Fliers are available in the office.

-Be sure to use the latest version of Adobe Acrobat to open this form and submit it properly. Here is a link to get the latest Adobe Reader:  http://get.adobe.com/reader/

The registration form should have a 'submit' button in the upper right corner. When you've completed the form, just click that button and it will automatically be emailed to us. If there is no button, complete the form and save it to your computer. Then email it as an attachment to registration@myorch.org. If you are using a Mac or otherwise have problems, please email the same address so we can make other arrangements.

If you have not completed a financial information form in 2012, please do so and get it to us ASAP for the sliding scale calculation. We cannot give your child a confirmed lesson time until we have calculated your per lesson fee and gotten your approval of the amount. 

You do not need to do this form if your children will only participate in the summer group classes: Summer Piano Sampler, Summer Orchestra Sampler, Rhythm Circle, Junior Choir.

The financial information form is NOT an online form. You must print it, complete it, and mail, fax or email it to us at the above address.
